Question : Point-to-point IP Phone

Is there a product on the market for an IP phone that does not require a gatekeeper?
For example, I have a client with an small offices (3 employees in each office) in IL and NY. The offices each have a DSL. They would like to have a "hot phone" that would have an ip address that they could call directly. There would be 1 phone in each office.
I know how to pass the H323 traffic through an overloaded NAT router to a single address. What I am looking for is a stand alone device (similar to a regular telephone) that if you dial the IP address of the other office, a similar phone on their end would ring.
I know I can use netmeeting or some other software based app but it would be nice to have a stand alone device. That way, you don't tie up a machine while you are on a call. I don't want to use gatekeepers or the like, just a single device on each end.
Any ideas??
