Question : New User Setup Request Forms / Access Request Forms

Is anyone willing to share their IT/HR Departments, tried and tested, New User Setup Forms and User Access Request forms for network access?

We currently have one in use, however form seems to be quite crude.

We are looking for one that may be more comprehensive that includes, but is not limited to, new user setup, access requests, assess level alterations, user deactivation when employment ceases, and perhaps an advanced warning or notification clause.  

Form should be simple to understand across various departments, easy to read / look at, and possibly able to be customized to suite changing applications, shares, and departments that may currently be in place in our organization.

Thanks for your help, but was looking for something more official.
We have sourced a workable template from another organizations IT Department in same industry.
